Key takeaways
- CMS-1500 is professional. UB-04 (also called CMS-1450) is institutional.
- Most practice claims go on the CMS-1500, usually sent electronically as an 837P.
- The UB-04 uses revenue codes and a type of bill, which the CMS-1500 does not.
- Which form a facility uses can depend on the payer, so confirm before billing.
What is the difference between the UB-04 and the CMS-1500?
The CMS-1500 is the standard paper form for professional claims. It is how physicians, therapists and many suppliers bill for their own services. The UB-04, also known as the CMS-1450, is the standard form for institutional claims, the charges a hospital or similar facility bills for a stay or visit.
Both forms have electronic equivalents, and almost all claims travel electronically today. The 837P carries professional claims and the 837I carries institutional ones.
Which form do you need?
Ask who is billing and for what.
- A physician billing for an office visit or procedure: CMS-1500 (837P)
- A hospital billing facility charges for an outpatient or inpatient stay: UB-04 (837I)
- A facility that also has physicians billing their own professional fees: both, as separate claims
Some facility types are paid on one form by one payer and the other form by another. Confirm with each payer rather than assuming.
What goes on each form
The CMS-1500 is organised into numbered items, and the UB-04 into form locators. Both carry patient and insurance details, diagnoses and the provider's identifiers. The differences are in how services are described:
- CMS-1500: procedure codes (CPT and HCPCS), modifiers, a place of service code and a rendering provider.
- UB-04: revenue codes that say which department of the facility provided the service, a type of bill code that describes the kind of claim, and facility-level details.
The National Uniform Claim Committee (NUCC) maintains the CMS-1500 and its instructions. The National Uniform Billing Committee (NUBC) maintains the UB-04. CMS publishes the CMS-1500 form as a PDF.
Common rejection causes
- Using the wrong form for the payer or the provider type
- A missing or mismatched provider identifier
- A revenue code that does not match the procedure on a UB-04
- A diagnosis that does not support the service billed
- Missing prior authorization or referral information
A clean claim is one a payer can process without sending it back. Catching these before submission is what claim scrubbing is for.
When this is not the right fit
If you are a small practice that bills entirely through an EHR that picks the form and builds the claim for you, you may never touch either form directly. And if you are a large hospital with a dedicated facility billing team, this guide is background, not a service need.
How Optimus handles claim forms
We scrub every claim before it goes out, whether it is professional or institutional, and we work inside your existing EHR or practice management system. See our medical billing services, our page on ambulatory surgery center billing or hospital billing, and read how an ERA explains the payment that follows.
